Bob Salvatore killed him off when he wrote a book for the series. I haven't read it, so you may have just actually mentioned the very book. He has been getting hate mail for it ever since.
In his own series for Forgotten Realms, he killed a Barbarian named Wulfgar. Wizards of the Coast (the parent of TSR) made him bring him back. He was resurrected a few books later.
